By clicking on the "Request prior military records form SF180" link, you can submit an online form SF-180 request direct to the Records Center for DD-214's, DD-215's, 201 Personnel File documents and Medical Records.  

You must submit a separate request for each set of documents you are requesting. Example, DD-214 or All Documents in your 201 File or document by Form number, Medical Records, or Replace all Medals.

Be very specific with your request and provide as much information as possible, this will help speed up the process considerably.
